Qualifications

About this position

North Memorial Health System is a certified Health Care Home (HCH) in which the primary care team works in partnership with customers to improve health related outcomes for individuals with chronic health conditions. Medication Therapy Management (MTM) is a unique service within HCH that is provided by a pharmacist in an ambulatory care setting to comprehensively assess a customer's medication regimen and to optimize medication therapy.

Requirements

Education

• Doctor of Pharmacy: Graduated from an accredited college of Pharmacy in 1996 or later OR have successfully completed MTM certification
• MTM residency preferred but will consider equivalent combination of Education and MTM Experience
• Board Certified Ambulatory Care Pharmacist (BCACP) highly desired

Experience

• 1+ year experience required in MTM residency or previous MTM position • Must possess excellent oral and written communication, leadership, self-initiative, innovative, analytical and collaborative skills
• Builds effective partnerships with providers and clinic staff fostering open lines of communication and establishing trust within the HCH team based care model
• Evaluates progress towards attainment of goals or outcomes
• Electronic Medical Record familiarity with EPIC is highly desired

Licensure/Certification

• Licensed by MN State Board of Pharmacy

Salary Range:N28 ($53.78 - $80.67), based on experience

Shift Differential: Compensation may include shift differential pay depending on the position and shift worked.
