Job summary

Qualifications
- Graduate of an ACPE accredited College of Pharmacy.
- Doctor of Pharmacy or other advanced degree.
- Completion of ASHP-accredited PGY-1 Pharmacy Residency with 3–5 years hospital experience.
- Eligible for licensure as a pharmacist in Illinois within 60 days of employment.
- Board Certification in area of expertise required within 18 months of hire.
Responsibilities
- Facilitate direct patient care programs and medication-use systems in general medicine and critical care units.
- Design and implement pharmacy programs, QA changes, and continuous quality assurance initiatives.
- Develop stewardship activities and surveillance programs for medication use.
- Track and evaluate programs for clinical, operational, and financial outcomes.
- Identify issues and lead clinical research, quality improvement, or education initiatives.
- Collaborate with multidisciplinary teams to optimize patient safety and outcomes.
