Cleveland Clinic Health System is seeking a Pharmacy Clinical Specialist in the DDI/Hepatology area on days at the Main Campus in Cleveland. This role coordinates pharmaceutical care and participates in patient rounds with medical staff to optimize pharmacotherapy.

The position requires a PharmD or a BS in Pharmacy with post-graduate residency/fellowship training. Certification preferences include BLS/ACLS and state licensure readiness for practice in Ohio.

Formación

• Doctorate or degree in Pharmacy and completion of post-graduate residency or fellowship required.
• If specialty area requires specialty residency, that residency is required.

Responsabilidades

• Coordinates and provides pharmaceutical care for patients on assigned clinical unit.
• Participates in patient care rounds with medical staff.
• Monitors pharmacotherapy and provides drug information to clinicians and patients.
• Leads staff education and supports departmental development.
• Supports research and continual learning in specialty areas.
• Educates pharmacists, technicians, students, and nurses in pharmacotherapy.
