Job summary

Wellstar is seeking a Clinical Pharmacy Specialist to provide comprehensive pharmaceutical care to patients through clinical services, formulary management, and policy development, including transitions of care from the hospital.

The role involves MTM, patient and provider education, and participation in pharmacy committees, with teaching responsibilities for students and residents. Requires PharmD or Doctorate, Georgia licensure, and 3+ years of clinical experience.

Qualifications

• PharmD or Bachelors in Pharmacy (preferred).
• Active Pharmacist license (RPH) required upon hire.
• Board certification (BCPS/BCCP) strongly preferred.

Responsibilities

• Provide comprehensive pharmaceutical care and MTM services.
• Manage therapy including anticoagulation, pharmacokinetics, and nutrition support.
• Round with interdisciplinary teams and document interventions.
• Clarify medications on admission and discharge; ensure safe transitions of care.
