Job summary

Legacy Community Health in Pasadena, TX is seeking a Clinical Pharmacy Specialist to provide direct patient care, manage chronic disease medications, and collaborate with a multidisciplinary team to optimize therapy.

This role includes patient counseling, medication management visits, precepting students, and contributing to quality improvement initiatives. Requires PharmD (or B.S. with experience), Texas license, ASHP PGY-1; PGY-2 and BCPS/BCACP are preferred.

Qualifications

  • PharmD or BS with relevant experience and licensure in Texas.
  • Completion of ASHP PGY-1 Pharmacy Residency is required.
  • Board-certified or eligible in pharmacotherapy/ambulatory care preferred.
  • Experience in HIV/geriatrics/pediatrics is desirable.
  • Ability to manage multiple deadlines and priorities.

Responsibilities

  • Provide pharmaceutical care through direct patient interaction and collaborative practice.
  • Conduct chronic disease medication management either face-to-face or virtually.
  • Obtain and summarize comprehensive medical histories and reconcile meds.
  • Document encounters in the electronic medical record and monitor outcomes.
  • Design and monitor therapeutic plans using appropriate medications.
  • Order labs/tests to evaluate efficacy and safety; report adverse events.
  • Educate patients, families, and providers about medications.
  • Precept pharmacy students and residents; participate in team care activities.
  • Work independently and as part of a collaborative care team.

Clinical Pharmacy Specialist - Job Overview

Key Responsibilities
  • Provide pharmaceutical care through direct patient interaction and collaborative practice.
  • Conduct face-to-face or virtual chronic disease medication management.
  • Obtain and summarize comprehensive medical and medication histories.
  • Reconcile patient medications during ambulatory care visits.
  • Perform physical assessments and triage patients as needed.
  • Document encounters and interventions in the electronic medical record.
  • Design, implement, and monitor therapeutic plans using effective and economical medications.
  • Manage medication therapy, including initiation, modification, renewal, and discontinuation.
  • Order labs and tests to evaluate medication efficacy and toxicity.
  • Document and report adverse drug events and medication allergies.
  • Review patient profiles for appropriate drug selection and dosage.
  • Provide drug information and education to patients, families, and providers.
  • Assess medication adherence during counseling.
  • Work independently and collaboratively, setting priorities effectively.
  • Serve as a preceptor to pharmacy students and residents.
  • Perform other duties as assigned.
Minimum Qualifications
  • Graduate of an accredited college/university of pharmacy (PharmD or B.S. with equivalent experience).
  • Completion of ASHP accredited PGY-1 Pharmacy Residency.
  • Valid license from the Board of Pharmacy of the State of Texas.
  • Certification in Basic Life Support / First Aid / CPR. Board-Certified Pharmacotherapy Specialist (BCPS) or Board-Certified Ambulatory Care Pharmacist (BCACP).
  • 2+ years of community pharmacy experience.
  • 2+ years of medication adherence counseling experience.
  • Experience in HIV/AIDs, Geriatrics, and/or Pediatrics.
  • Completion of ASHP accredited PGY-2 Pharmacy Residency in Ambulatory Care, Adult Medicine, Geriatric Medicine, or Pediatrics.
  • Certification in immunization delivery or smoking cessation.
  • Computer experience Windows, Microsoft Word & Excel.
  • Must be able to manage multiple deadlines and prioritize.
  • Excellent communication skills and customer service skills.
