Dragonfly Health is seeking a Clinical Pharmacist to provide clinical review and consultation on medication requests within hospice and post-acute care. You will evaluate dosing, safety, and effectiveness using evidence-based guidelines, collaborating with providers to optimize patient outcomes.

Qualifications

• Licensed pharmacist in good standing in PA, TN, or OH or eligible.
• 1+ year of pharmacy experience preferred (retail, hospital, or managed care).
• Solid knowledge of formulary management and PBM structures.
• Strong written and verbal communication.

Responsibilities

• Provide clinical review and consultation on medication requests, prior authorizations, and benefit determinations.
• Evaluate therapy appropriateness, dosing, and safety based on guidelines.
• Collaborate with providers to clarify prescriptions and suggest alternatives when appropriate.
• Support complex case reviews and appeals with accurate clinical documentation.
• Identify opportunities to improve medication adherence, safety, and cost-effectiveness.
• Educate internal teams on formulary management and pharmacy policy updates
