Job summary

  • Assist with routine maintenance of clinical areas and equipment.
  • Manage time, tasks, and resources to meet duties.
  • Represent BRMC professionally and promote its quality care and patient-centered service.
  • Perform other duties as assigned by the Clinical Pharmacist to meet care goals.

Job Summary

The Clinical Pharmacy Care Coordinator will function in conjunction with the clinical pharmacy team and the Quality Team to deliver care within a Federally Qualified Health Center. Duties include performing as a reliable, self-motivated member of the team and assisting in the administration and maintenance of an efficiently operated clinic. This role is essential in promoting preventive care, addressing health disparities, and improving outcomes for patients. The Clinical Pharmacy visits currently consist of Medicare Annual Wellness Visits, but may be expanded to include acute visits or focused clinics. This position may involve assignments at satellite locations as needed.

Essential Job Duties
  • Conduct outreach to schedule clinical pharmacy visits and other visits as necessary, prioritizing high-risk patients by using appropriate portals and insurance information.
  • Conduct pre-visit planning for efficient visits.
  • Complete standardized rooming and perform clinical intake, data collection, and documentation of patient care provided. This may include administering and documenting standardized screening tools (depression, fall risk, cognitive assessment, social determinants of health).
  • Administer immunizations under the direction of a provider.
  • Assist in examination and treatment of patients. Perform necessary test(s) as directed by provider. [KB1]
  • Collect, label, and prepare specimens for lab. [KB2]
  • Support accurate, timely, and thorough clinical documentation, appropriate coding practices, and compliance with quality reporting requirements.
  • Assist with routine maintenance activities including, but not limited to, the following: cleaning equipment, stocking exam rooms, organizing clinical work area, etc.
  • Complete work activities successfully through effective management of time, tasks and available resources. Remain vigilant in the performance of duties.
  • Represent the organization in a professional manner through appropriate appearance, respectful communication, and compassionate interactions. Demonstrates the ability to articulate and promote the value of the healthcare services provided, including the organization’s commitment to quality care, patient-centered service, and improved health outcomes for the community.
  • Perform other necessary duties as assigned by the Clinical Pharmacist to meet the goal of providing quality health care services.

Compensation Range: $20.76-$24.11/hr

Position Requirements
Minimum Qualifications
  • Qualified candidates may include Certified Medical Assistants, Licensed Practical Nurses (LPNs), or Registered Nurses (RNs) with appropriate clinical experience. Responsibilities will be performed based on the individual’s training, credentials, and applicable scope of practice.
  • Current CPR certification
  • Experience with case management or care coordination highly recommended.
  • Experience with patient assistance programs, or community health preferred.
  • Excellent communication skills (both written and verbal), computer skills and proficiency in using an electronic health records system for charting, scheduling and communication.
  • Ability to speak fluently in Spanish a plus.
Required Skills

Must be able to interact successfully with patients and all levels of employees and providers in a multidisciplinary setting; must be able to work courteously and effectively as a team member and also independently; demonstrate strong attention to detail; must maintain confidentiality; ability to prioritize and manage multiple projects; and must have knowledge of and ability to work with computer programs (MS Office, EHR, etc.) and office equipment used in clinical settings. Must be able to adapt to changes as the clinical pharmacy visits evolve.

Working Conditions
  • This position is a FLSA non-exempt position which may require work in excess of 40 hours per week. Work hours are Monday through Friday during variable hours of clinical operations to meet the Center’s needs and accomplish the duties of this position.
  • Comply with and set example of compliance with BRMC’s policies/procedures/practices; comply with legal requirements for non-profit organizations.
  • Perform and complete duties under stress and demands from different sources, and meeting strict deadlines.
  • Tolerate prolonged computer related exposure. Able to sit or stand at a workstation for long periods of time.
  • May be exposed to hostile or emotional clients, family members, and staff, as well as disturbing or sensitive information
  • May be exposed to infectious diseases and medical waste.
