Job summary

UCHealth is seeking a Clinical Pharmacist - Pharmacy Transitions of Care in Aurora, CO. The role focuses on optimizing medication therapy and safe transitions from hospital to community, with primary work at 2450 S. Peoria Street and occasional on-site at Anschutz Medical Campus.

It is a full-time, onsite position with 80 hours per pay period (2 weeks), typically Monday–Friday, 8:00 AM–4:30 PM. The TOC Pharmacist reviews admission medication histories, resolves barriers, and conducts discharge

Qualifications

• Bachelor's Degree in Pharmacy required.
• State licensure as a Pharmacist required.
• Relevant life support certification as determined at position level.

Responsibilities

• Review, interpret, and evaluate medication regimens to identify, prevent, and resolve medication-related problems.
• Identify and address medication access, affordability, adherence, and other barriers during transitions from hospital to community.
• Perform discharge medication reconciliation to resolve discrepancies and promote safe medication use following discharge.
• Collaborate with providers and care team to develop, implement, and evaluate medication management plans.
• Review medication histories and provide oversight, guidance, and feedback.
• Communicate with patients and care team to resolve medication-related concerns.
• Provide medication education and counseling to patients when appropriate.
• Participate in quality improvement initiatives related to transitions of care.
