UW Medicine Montlake Pharmacy Department seeks a Clinical Pharmacist for Surgical Intensive Care and General Surgery to join the inpatient team. The role focuses on optimizing pharmacotherapy for surgical patients and participating in protocol development and education of staff.

Applicants should have an advanced degree, residency, or two years of hospital pharmacy experience; PGY-2 critical care training is preferred. Licensure in Washington is required.

Formación

• Two years of pharmacy practice experience.
• ASHP-accredited residency training or advanced degree preferred.
• Board-eligible or board-certified clinical pharmacist experience in hospital settings a plus.

Responsabilidades

• Ensure appropriate drug therapy and nutrition, including indication, dose, interactions, and allergies.
• Evaluate pharmacotherapeutic regimens for effectiveness and adverse events; intervene as needed.
• Document pharmacotherapeutic recommendations in the medical record and communicate with care teams.
• Provide drug information and education to medical, nursing, and dietitian staff; counsel patients and families.
