Job description

Job Summary

Oversees the provision of safe and effective medication therapies through collaboration with the patient’s care team. Reviews physician orders for drug therapy and monitors this therapy for drug choice, dosage, drug interactions, adverse drug reactions, and formulary adherence. Serves as a drug information resource to nurses, physicians, and other healthcare workers.

Essential Functions

  1. Retrieves and evaluates relevant biomedical literature, formulates an appropriate response, and provides concise and timely responses to drug information requests.
  2. Uses effective patient education techniques to provide counseling to patients and caregivers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration.
  3. Provides accurate shift-to-shift communication regarding unresolved situations requiring future action.
  4. Collects information needed to design medication therapy regimens and identify medication-related problems with current therapies.
  5. Designs, recommends, monitors, and evaluates patient-specific pharmacotherapeutic regimens. Implements recommendations effectively and documents in the medical record using the appropriate format, following policies and procedures.
  6. Participates in the management of medical emergencies.
  7. Precepts pharmacy students and pharmacy residents.
  8. Prepares and dispenses medications in accordance with best practices, policies, and state and federal regulations. Manages medication therapies using clinical guidelines.
  9. Checks the accuracy of the work of pharmacy support team members or others in accordance with the organization’s policies and procedures.

Physical Requirements

Works primarily in the pharmacy, delivering medications to nursing units. Requires considerable walking, standing, pushing a mobile cart (up to 50 pounds), and lifting 25 pounds of material. May frequently need to stretch, bend, squat, or kneel. Uses appropriate PPE to minimize exposure to hazardous agents and takes precautions against needle sticks, cuts, or bruises. Exposed to conditions such as walking on hard surfaces, climbing stairs, and temperature/humidity changes. Must perform manipulative skills like writing, typing, and data entry. Requires good vision to read small print and effective communication skills. Involves rotating shifts.

Education, Experience, and Certifications

Bachelor's Degree in Pharmacy or PharmD from an accredited school required. Must be licensed in North Carolina or eligible for reciprocity. Prior hospital pharmacy experience preferred. Must have excellent verbal communication and interpersonal skills. Immunization Certification required.
