Qualifications

• Doctorate of Pharmacy required; BPS Certification within 3 years of start date.
• Two years of residency experience or one year of residency plus three years of work experience; equivalency accepted.
• Colorado DORA Pharmacy license required.
• BLS certification (American Heart Association, American Red Cross, or American Safety and Health Institute accepted).
• Preference for heart transplant or transplant PGY2 experience.

Responsibilities

• Designs and leads pharmaceutical outcomes research; participates in drug utilization reviews and research.
• Educates pharmacy staff, nursing staff, medical staff, residents and students; serves as a preceptor.
• Provides drug information services and communicates literature-based recommendations to patient care teams; helps develop medication-use policies.

The Clinical Pharmacist Specialist ensures safe, appropriate, and economical use of drugs through the application of specialized skills and knowledge in pediatric patient care.

Department: Pharmacy-Heart Transplant Team

Shift: M-F dayshift with every 3rd weekend and holiday rotation

Children's Hospital Colorado is home to one of the nation's top pediatric heart transplant programs, bringing together a highly collaborative, multidisciplinary team of pharmacists, physicians, advanced practice providers, nurses, social workers, dietitians, transplant coordinators, and other specialists dedicated to delivering exceptional care. Team members work closely across disciplines to support patients and families throughout the transplant journey, creating a dynamic environment that values partnership, innovation, and shared expertise.

With more than 550 pediatric heart transplants performed, Children's Colorado is one of the most experienced pediatric heart transplant centers in the country. The program consistently achieves outcomes that exceed national benchmarks, including a one-year patient survival rate of 97.78% and a three-year survival rate of 96%. As part of this nationally recognized team, the Heart Transplant Clinical Pharmacist Specialist will play a key role in advancing patient care, contributing to program excellence, and helping shape the future of pediatric heart transplantation.

Duties & Responsibilities

• Designs and leads pharmaceutical outcomes research. Identifies opportunities for and participates in drug utilization reviews and research. Identifies opportunities for publishing case reports, unique therapeutic interventions, development of practice standards, and other medication related pharmacy topics.
• Participates in the education of pharmacy staff, nursing staff, medical staff, pharmacy residents and students, other healthcare allied professionals and patients and families. Serves as a preceptor and mentor for pharmacy residents and students.
• Provides drug information services by responding to questions regarding medication compatibilities, adverse drug events, regimens, dosing, interactions, storage and disposal, compounding, therapeutic uses of medications and pharmaceutical supplies in a timely manner; using a systematic approach to effectively searching, retrieving, and evaluating the literature and appropriately communicating and applying the information to the patient care situation; and assisting with the development, maintenance, and implementation of hospital and department policy and procedures related to medication use, storage, restrictions, and administration.
• Sustains and implements quality and safety initiatives and practices in the department and throughout the hospital. Reports adverse drug events and medication variances using the hospital variance reporting program (QSRS). Demonstrates sustained record of identifying and leading quality improvement/assurance initiatives.
• Sustains clinical pharmacy services throughout the organization by providing pharmaceutical care for pediatric patients of all age ranges through participation in multi-disciplinary team rounds and/or care, emergency response, education, guidance, interpretation and analysis of medication related therapeutic issues; documenting pharmaceutical interventions according to departmental standards; performing therapeutic drug monitoring and disease outcome analysis; and timely verification of medication orders in an electronic health record system.
• Participate in general pharmacy operations throughout the organization through appropriate medication preparation, dispensing, and inventory control. Provides oversight to pharmacy technicians, interns, residents and students as needed.

Minimum Qualifications

• Degrees: BS in Pharmacy or
• Doctorate of Pharmacy
• Experience: Two (2) years of residency experience or
• One (1) year of residency plus three (3) years of work experience
• Equivalency: Three (3) years of pharmacy work experience in area of specialty accepted in lieu of 2nd year of residency.
• Licenses & Certifications: Board of Pharmacy Specialties (BPS) Certification required within 3 years of start date in position
• BLS certification (American Heart Association, American Red Cross, or American Safety and Health Institute accepted)
• State of Colorado Department of Regulatory Agencies (DORA) Pharmacy license
• Preference for experience in heart transplant or transplant specific PGY2

Salary Information

Pay is dependent on applicant's relevant experience.

Annual Salary Range (Based on 40 hours worked per week): $130,499.55 to $195,749.32
