Job summary

CommonSpirit Health is seeking a Clinical Pharmacist Specialist Ambulatory Services to join the ambulatory specialty care team. You will collaborate with physicians, APPs, and clinic staff to optimize therapy for patients with complex chronic diseases and to grow specialty pharmacy services.

You will coordinate medication access, prior authorization support, monitoring, immunization assessment, and ongoing follow-up, while developing workflows and best practices for the evolving program.

Qualifications

  • Doctor of Pharmacy degree required.
  • 3 years of direct patient care in ambulatory care.
  • Nebraska Pharmacist License required.

Responsibilities

  • Collaborate with physicians, APPs, and clinic staff to optimize medication therapy.
  • Coordinate specialty medication management including access and prior authorization support.
  • Monitor laboratory results and medication safety.
  • Provide immunization assessment and patient education.
  • Conduct ongoing clinical follow-ups with patients in collaboration with the care team.
  • Support development of workflows and best practices for the specialty pharmacy program.

Job Summary And Responsibilities

As our Clinical Pharmacist Specialist Ambulatory Services, you will serve as an integral member of the ambulatory specialty care team, partnering with physicians, advanced practice providers, and clinic staff to optimize medication therapy for patients with complex chronic diseases. You will also help support the development and growth of specialty pharmacy services while delivering safe, evidence-based, patient-centered care.

Every day you will work closely with patients, specialty pharmacies, and interdisciplinary healthcare professionals to facilitate timely therapy initiation, improve medication adherence, monitor treatment response, and identify and resolve medication-related problems. You will coordinate specialty medication management activities including medication access, prior authorization support, laboratory and safety monitoring, immunization assessment, patient education, and ongoing clinical follow-up in collaboration with the care team.

You Will Be Expected To
  • Partner with physicians, advanced practice providers, and clinic staff to optimize medication therapy.
  • Coordinate specialty medication management activities, including medication access and prior authorization support.

  • Adapt processes to meet the needs of patients, providers, and the organization.
  • Demonstrate flexibility, initiative, and collaboration while helping shape the future of ambulatory specialty pharmacy services.
  • Support the continued growth and success of the specialty pharmacy program.
Job Requirements
Required
  • Doctor of Pharmacy
  • Clinical experience with 3 years of recent direct patient care in the ambulatory care setting
  • Nebraska Pharmacist License
