Qualifications

• Pharmacy degree from an accredited school of pharmacy.
• Registered Pharmacist with a state license in good standing.

Responsibilities

• Support quality measures for Essential Plan and Medicare populations.
• Conduct provider and member outreach to address gaps in care.
• Assist with medication adherence initiatives and CMS/NYSDOH compliance.
• Precept Pharmacy Residents and Students.
• Review monthly reports and provide feedback on trends and efficiency.
