Job summary

Glendale Memorial Hospital and Health Center is seeking a Clinical Pharmacist to review prescriptions for safety and efficacy, and to ensure accurate dispensing and documentation. You will work closely with doctors and nurses to optimize patient care and outcomes.

You will interpret orders, verify medications, educate patients, and participate in real-time problem solving to maintain high-quality care for all age groups served.

Qualifications

• Pharmacist with CA license is required.
• Doctor of Pharmacy degree required.
• Active CA Pharmacist license and BLS CPR certification.
• ACLS certification preferred.

Responsibilities

• Review prescriptions for drug interactions, dosing, allergies, and safety.
• Verify and dispense medications and ensure timely labeling and entry.
• Communicate drug information to patients and health professionals.
• Collaborate with providers to optimize drug therapy and outcomes.
• Document clinical activities and interventions per policy.
