Job summary

Qualifications

  • PharmD degree and NC license required.
  • CPP license within 1 year of hire preferred.
  • Residency training (PGY1) preferred; PGY2 ambulatory care preferred.
  • Board certification (BCPS) or equivalent preferred.

Responsibilities

  • Develop training programs to support system changes.
  • Provide medication therapy management, symptom management, and chemotherapy/medication education per protocol.
  • Serve as a pharmacy resource with literature searches and drug information inquiries.
  • Maintain professional relationships with CFVHS staff.
  • Participate in organizational improvement activities.
  • Complete all required documentation.
  • Precept PGY1 residents.
  • Precept PharmD students.
  • Hold self and staff accountable for patient service.
  • Develop and implement quality improvement measures.

Works as a Clinical Pharmacist Practitioner for Cape Fear Valley Health System.

Major Job Functions
  • Develop training programs to support system changes
  • Provides medication therapy management, symptom management, and chemotherapy / medication educations as allowed per the protocol between the CPP and the physicians of the CFV clinic.
  • Serves as pharmacy resource to providers in a CFV clinic to include but not limited to: primary literature search, drug information inquiries, staff continuing education presentations.
  • Maintains effective professional relationships with CFVHS staff.
  • Participates in organizational improvement activities.
  • Completes all required documentation.
  • Serve as member of Residency Advisory Committee.
  • Develops working relationships with outpatient physicians, physician assistants, nurse practitioners, nurses, and clinic directors in the Cape Fear Valley system.
  • Administers vaccinations and demonstrates appropriate medication administration techniques.
  • Precepts, evaluates, and mentors PGY1 pharmacy residents.
  • Precepts PharmD students.
  • Holds self and staff accountable for providing exemplary patient service.
  • Develops and implements quality improvement measures to improve patient care and staff efficiency.
  • Other duties as assigned.
Minimum Qualifications

The following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this job:

Education and Formal Training:
  • Doctorate of Pharmacy from an accredited school of pharmacy required.
  • License with the North Carolina Board of Pharmacy required.
  • Obtain Clinical Pharmacist Practitioner (CPP) license within 1-year of job acceptance.
  • PGY1 pharmacy residency preferred.
  • PGY2 in ambulatory care pharmacy or equivalent ambulatory care experience preferred.
  • Board Certification preferred, Board-Certified Pharmacotherapy Specialist (BCPS) or other
Work Experience:
  • 2 years of pharmacy practice experience required.
  • Experience in precepting pharmacy students and/or residents preferred.
Knowledge, skills, abilities required:
  • Proficiency in reading, writing, and speaking the English language
  • Must be an effective communicator and independent thinker.
  • Demonstrates ability to manage computer systems.
  • Needs to possess problem solving skills and natural ability to manage staff.
  • Demonstrates strong background in clinical knowledge and can effectively communicate with patients.
  • Possesses the ability to communicate effectively to providers in an interdisciplinary setting.
  • Constantly able to evolve and flourish as the world of ambulatory care pharmacy necessitates
Physical requirements:
  • Ability to freely move throughout pharmacy, clinics, and surrounding community to interact with staff and perform duties.
  • Ability to hear conversations with ease and read orders correctly.
  • Ability to effectively communicate
  • Ability to move 5 pounds.
