Job description

UNC Health is seeking a highly motivated pharmacist to join the UNC Adult Neurology Clinic. The role is for an immediate opening for a Clinical Pharmacist Practitioner (CPP) who will play an integral role in the care of patients within the Neurology Division, supporting the Rapid Access Neuroimmunology and Neuromuscular program.

Summary

The Neurology CPP will serve as an Advanced Practice Provider on the interdisciplinary health care team and will provide clinical support to neurologists, nurses, and other ancillary staff. The position focuses on rapid access to care for patients with complex neurological conditions, with an emphasis on avoiding unnecessary emergency department visits and hospital admissions.

Responsibilities
  • Conduct pharmacotherapy visits, manage high-cost and complex medication regimens, coordinate specialty therapies, and provide longitudinal medication monitoring.
  • Coordinate care transitions between inpatient, outpatient, and infusion settings, collaborating closely with specialty pharmacy, infusion services, and care coordination teams.
  • Provide educational mentorship and experiential precepting to pharmacy students, PGY1 and PGY2 residents, and neurology trainees.
  • Partner with the Department of Pharmacy and Neurology leadership to support quality improvement initiatives focused on access, care coordination, and clinical outcomes.
  • Engage in scholarly activities including research, manuscript preparation, and committee participation.
  • Lead continuous quality improvement projects and develop new clinical programs or services within the clinic/department.
Education Requirements
  • Bachelor's degree in Pharmacy.
Licensure/Certification Requirements
  • Registered Pharmacist licensed as a CPP in the state of North Carolina, with credentialing and privileging completed through the UNC HCS accrediting body as an APP prior to the start date.
Professional Experience Requirements
  • North Carolina registered Pharmacist: BS Pharmacy or Pharm D. BPS Certification or Geriatric Certification or ASHP residency program + 2 years of experience.
  • North Carolina Registered Pharmacist: Pharm D. 3 years experience. 1 certificate program.
  • North Carolina Registered Pharmacist: BS Pharmacy. 5 years experience. 2 certificate programs.
