Job summary

Penn Highlands Healthcare seeks a Clinical Pharmacist to participate in daily rounds, review patient profiles, and make recommendations to physicians to optimize pharmacotherapy. Pharmacokinetic monitoring is a core daily activity within a decentralized pharmacy service model.

The role includes liaising with other departments, precepting pharmacy students, and delivering medication education as part of a collaborative care team.

Qualifications

• Graduate from School of Pharmacy is required.
• One year in a hospital pharmacy environment is preferred.
• PGY-1 residency or equivalent experience in clinical pharmacy practice is preferred.
• Licensed to practice pharmacy in the Commonwealth of Pennsylvania is required.

Responsibilities

• Attend daily rounds and review patient profiles to provide pharmaceutical care.
• Make recommendations to physicians and extenders to optimize drug therapy.

Summary:
