Job summary

Saint Clare's Health, part of Prime Healthcare, seeks a licensed pharmacist to join a hospital-based team delivering high-quality pharmaceutical care across medicine, ICU, obstetrics, pediatrics, and ambulatory services. The role emphasizes direct patient care, rounding, and interdisciplinary collaboration to optimize pharmacotherapy.

The position requires a PharmD or equivalent, with PGY-1/PGY-2 training and BLS; ACLS is a plus, and background in hospital pharmacy operations is preferred.

Qualifications

  • Current state Registered Pharmacist License.
  • B.S. degree in Pharmacy required; Doctor of Pharmacy degree preferred.
  • Completion of PGY-1 or >3 years of qualifying experience required.
  • PGY-2 postgraduate training preferred.
  • Board certification in a Pharmacy specialty preferred.
  • BLS Required
  • ACLS Preferred
  • Experience in hospital pharmacy operations preferred.

Responsibilities

  • Coordinate the activities of the pharmacy department to provide high-quality pharmaceutical care for all patients served.
  • Spend 50%+ of professional time in direct patient care including rounding, patient counseling, or committees participation.
  • Provide direct patient care including observation and evaluation of medication-related needs and pharmacotherapy adjustments.
  • Collaborate with other health professionals for ongoing pharmacotherapeutic monitoring and follow-up across units such as General Medicine, ICU, Ob/Gyn, Pediatrics, Emergency, Oncology, Peri-operative, and Outpatient Services.

Responsibilities

Under the supervision of the Director of Pharmacy and clinical coordinator (if applicable), coordinates the activities of the pharmacy department to provide the highest quality of pharmaceutical care to all patients served by the hospital, while ensuring patient safety. Conducts 50% or more of professional time in direct patient care activities including but not limited to interdisciplinary rounding, patient counseling, or participation in patient care committees. Provision of direct patient care encompasses activities such as the direct observation and evaluation of the patient and his/her medication-related needs; the initiation, modification, or discontinuation of patient-specific pharmacotherapy; and the ongoing pharmacotherapeutic monitoring and follow-up of patients in collaboration with other health professionals. The scope of patient care provided may include: General Medicine/Surgical, Intensive Care Services, Obstetrics & Gynecology, Pediatrics, Emergency Care, Oncology, Peri-operative, Geriatrics and Outpatient Services.

Qualifications
  • Current state Registered Pharmacist License.
  • B.S. degree in Pharmacy required; Doctor of Pharmacy degree preferred.
  • Completion of postgraduate clinical training (PGY-1) or > 3 years of qualifying experience required.
  • Postgraduate clinical training PGY-2 preferred.
  • Board certification in a Board of Pharmacy Specialties clinical specialty preferred.
