Qualifications

• Doctor of Pharmacy Degree (PharmD) required.
• Residency training (1–2 years) and/or at least two years of clinical pharmacy work experience.
• Pharmacist License with the Texas State Board of Pharmacy in good standing; BPS certification preferred.
• Ability to set priorities, work under pressure, communicate effectively, and work independently.

Responsibilities

• Provide pharmacotherapy support to medical staff and providers to optimize medication selection, dosing, and monitoring.
• participate in multidisciplinary rounds and collaborations to optimize pharmacotherapeutic care.
• Apply specialized knowledge to evaluate the appropriateness and effectiveness of patient medications.
• Contribute to Antimicrobial Stewardship by ensuring appropriate antimicrobial therapy and duration.
• Educate nursing, medical staff, and pharmacy staff on therapies and safety.
