Job summary

Jefferson is seeking a Clinical Pharmacist to oversee the medication distribution system in its service area. The role focuses on ensuring each medication is indicated, effective, safe and convenient for patients, with emphasis on pharmacokinetic dosing and therapy optimization.

The position requires PharmD and licenses in PA and NJ, with responsibilities including staff training, precepting students, and contributing to quality and safety initiatives within TJUH.

Qualifications

  • Knowledge of pharmacokinetic principles for dosing.
  • Monitor patients for response to medications.
  • Identify and document sentinel/serious events, near misses, medication errors and ADRs in the EHR.
  • Promote and assure compliance with hospital formulary and policies.
  • Provide care appropriate to patient age and background.

The Clinical Pharmacist oversees the medication distribution system for their area of responsibility. In addition, the Clinical Pharmacist identifies, prevents, and resolves drug therapy problems by ensuring each medication is indicated, effective, safe, and convenient for the patient.

Job Duties
  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson. (Do not delete, move or overwrite this statement)
  • Assures the appropriateness of drug therapy and compliance with hospital policies
  • Oversees the distribution of medications throughout TJUH
  • Supports implementation of processes to assure the quality of pharmacy services
  • Trains staff as assigned (e.g., residents, interns, technicians, new pharmacists)
  • Precepts student pharmacists as assigned. See Preceptor Development & Evaluation Program policy.
Minimum Qualifications
  • Doctorate Doctor of Pharmacy or RPH (see below under experience)
  • Knowledge of pharmacokinetic principles required for medication dosing
  • Monitor patients for response to medications
  • Identify and document sentinel/serious events, near misses, medication errors and adverse drug reactions in the electronic reporting system
  • Promote and assure compliance with the hospital formulary and policies and procedures defined by the P & T Committee
  • Provide care that is appropriate to the chronological age, developmental age, social maturity, and cultural background of the patient
  • RP - Registered Pharmacist - State of Pennsylvania
  • RP - Registered Pharmacist - State of New Jersey
Preferred Qualifications
  • 1 year PGY1 Pharmacy Residency preferred
  • Microsoft Word and Microsoft Outlook skills
Additional Qualifications
  • Certification required based on primary work location.
Salary Range

$53.49 to $76.22 Hourly. The actual hiring rate will be determined based on candidate experience, skills and qualifications. This position is not eligible for an annual incentive. Positions posted as annual salaries reflect an estimated amount for a 1.0 Full-Time Equivalent (FTE) (i.e. 40 hour equivalent), positions for less than 1.0 FTE will be prorated.
