Job summary

Jefferson Health’s Albert Einstein Medical Center is seeking a Clinical Pharmacist to oversee medication distribution and resolve drug-therapy problems for their service area. You will ensure medications are indicated, effective, safe, and convenient for patients.

The role emphasizes pharmacokinetic dosing, monitoring patient responses, and adherence to hospital formulary with a focus on patient-centered care.

Qualifications

  • Doctor of Pharmacy or RPH degree.
  • Knowledge of pharmacokinetic principles required for medication dosing.
  • Monitor patients for response to medications.
  • Identify and document sentinel/serious events, near misses, medication errors and adverse drug reactions in the electronic reporting system.
  • Promote and assure compliance with the hospital formulary and policies and procedures defined by the P & T Committee.
  • Provide care that is appropriate to the chronological age, developmental age, social maturity, and cultural background of the patient.
  • RP - Registered Pharmacist - State of Pennsylvania or RP - Registered Pharmacist - State of New Jersey.

Minimum Qualifications
  • Doctorate Doctor of Pharmacy or RPH (see below under experience)
