Clinical Pharmacist

Providence St Vincent Medical Center in Portland, OR – Per Diem/Variable Shift.

Responsibilities

The Clinical Pharmacist is responsible for providing high-quality, individualized, cost‑effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient medication therapy outcomes and quality of life.

Required Qualifications

• PharmD, Pharmacy degree, or bachelor’s degree in pharmacy
• Oregon Pharmacist License upon hire

Preferred Qualifications

• Acute‑care hospital pharmacy experience, 1 year or equivalent
• Completion of an ASHP‑accredited PGY1 residency

Posted wage range: $72.42 – $114.33 per hour. The final placement within this range will be based on relevant experience and other applicable factors.
