Job summary

Cleveland Clinic Health System is seeking a Clinical Pharmacist for the Pediatric Inpatient unit at the Main Campus in Cleveland. The role focuses on safe preparation, verification, and distribution of medications for hospitalized pediatric patients and supports daily inpatient operations.

The caregiver will work variable weekday shifts, participate in weekend rotations, and may assist with infusion services. A commitment to patient-centered, evidence-based care is essential.

Qualifications

  • PharmD or Bachelor’s of Science in Pharmacy from an accredited college of Pharmacy.
  • Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation within 90 days of hire.
  • BLS through AHA or Red Cross and completion of an immunization administration course within 90 days of hire.

Responsibilities

  • Provide safe, patient-centered, evidence-based medication services.
  • Verify medication orders and dispense according to regulations.
  • Document services completely and timely in the patient record.
  • Contribute to medication safety initiatives and compliance with accreditation standards.
  • Direct staff to provide safe care and maintain professional relationships.
  • Educate and train students, residents, and caregivers/patients.

Education

The Clinical Pharmacist – Children's provides specialized pharmacy support within Cleveland Clinic Children's at Main Campus, with a primary focus on Pediatric Inpatient Pharmacy Operations. This role is responsible for ensuring the safe and accurate preparation, verification, and distribution of medications for hospitalized pediatric patients while supporting daily inpatient pharmacy operations. Key responsibilities include verifying medication orders, answering medication-related inquiries from clinical staff, collaborating closely with nursing and interdisciplinary care teams, and assisting with operational and clinical problem-solving throughout the shift. The pharmacist also serves as the secondary responder for pediatric code situations and may have opportunities to support infusion services based on departmental needs and individual interest.

A caregiver in this position will work variable weekday shifts based on departmental needs, including 6:30am–3:00pm, 7:00am–3:30pm, 7:30am–4:00pm, 8:00am–4:30pm, and 1:00pm–9:30pm. Caregivers participate in an every-eighth-weekend rotation, with weekend shifts typically scheduled 6:30am–3:00pm or 1:00pm–9:30pm, and work one rotating holiday each year.

A caregiver who excels in this role will:

  • Provide safe, efficient, patient-centered, evidence-based medication services.
  • Verify medication orders and/or dispenses medications accurately according to state and federal regulations.
  • Document services in a complete,accurateandtimelymanner.
  • Actively contribute to medication safety initiatives.
  • Ensure all pharmacy operations are in compliance with national accreditation standards.
  • Directs staff appropriately to provide safe and efficient care andmaintainprofessional working relationships with all caregivers.
  • Educate and train students, residents, othercaregiversand/or patients.

Minimum qualifications for the ideal future caregiver include:

  • PharmD orBachelor’sof Science in Pharmacy from an accredited college of Pharmacy
  • Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within90 daysof hire
  • For individuals required to administer immunizations,Basic Life Support (BLS) through American Heart Association (AHA) or American Red Cross andcompletion of a Board of Pharmacy approved course inthe administration of immunizations or certificationobtained within 90 days of hire are required.

Preferred qualifications for the ideal future caregiver include:

  • Board Certification
  • Completion of a post-graduate year 1 residency (PGY1), fellowship or relevant post-graduate pharmacy experience preferred.
  • Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area
  • Inpatient experience strongly preferred

Physical Requirements:

  • Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.
  • Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.
  • Position often includes reaching, stretching, stooping, climbing steps and sitting.
  • Must have normal or corrected vision.
  • Must have ability toclearly communicate verbally by phone or in person
