Cleveland Clinic South Pointe Hospital in Warrensville Heights, OH is seeking a PRN Clinical Pharmacist for inpatient services. The role focuses on rounding with the multidisciplinary team, reviewing and optimizing pharmacotherapy, and supporting both decentralized and centralized pharmacy operations.

The ideal candidate will hold PharmD or B.S. in Pharmacy, be a licensed RPh, and possess relevant certifications (immunization, ACLS/PALS). Prior teaching or precepting experience is a plus.

Qualifications

  • PharmD or B.S. in Pharmacy from an accredited college of pharmacy.
  • Current state licensure as a Registered Pharmacist or eligible for reciprocity within 90 days of hire.
  • Immunization administration certification (BLS) required or obtained within 90 days of hire.
  • ACLS or PALS certification required within 1 year of hire.
  • Board certification is preferred.

Responsibilities

  • Round with interdisciplinary care teams to optimize medication therapy for hospitalized patients.
  • Provide clinical pharmacy support and collaborate with providers and nursing staff.
  • Assist with centralized pharmacy operations as needed and precept pharmacy students and learners.

A caregiver in this role will work PRN supporting both decentralized and centralized pharmacy operations at South Pointe Hospital. This Clinical Pharmacist will primarily support Internal Medicine services by rounding with the interdisciplinary care team, reviewing and optimizing medication therapy, collaborating with providers and nursing staff, and providing clinical pharmacy support for hospitalized patients. Additional responsibilities include assisting with centralized operations as needed and supporting pharmacy education through precepting pharmacy students and learners.

This is a PRN position. A caregiver in this role will work days/evenings including 1 holiday per year and every 5th weekend.

A caregiver who excels in this role will:

  • Provide safe, efficient, patient-centered, evidence-based medication services.
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.
  • Document services in a complete,accurateandtimelymanner.
  • Ensure all pharmacy operations comply with national accreditation standards.
  • Maintain professional interactions with patients tofacilitatesuccessful patient experiences.
  • Educate and train students, residents, other caregivers, and/or patients.

Minimum qualifications for the ideal future caregiver include:

  • PharmD or Bachelor of Science degree in Pharmacy from an accredited college of pharmacy.
  • Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within90 daysof hire.
  • For individuals required to administer immunizations,currentBasic Life Support (BLS) through American Heart Association (AHA) or American Red Crosscertification andcompletion of a Board of Pharmacy approved course inthe administration of immunizations or certificationobtained within 90 days of hire are required.
  • For individuals required to respond to adult or pediatriccardiopulmonary arrest events, currentAdvancedCardiovascular Life Support (ACLS) or PediatricAdvanced Life Support (PALS)Certification through American Heart Association (AHA), orcertification obtained within one (1) year of hire, is required.

Preferred qualifications for the ideal future caregiver include:

  • Board certification.
  • Completion of a post-graduate year 1 residency (PGY1), fellowship or relevant post-graduate pharmacy experience preferred.

  • Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area

Physical Requirements:

  • Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.

  • Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.

  • Position often includes reaching, stretching, stooping, climbing steps and sitting.

  • Must have normal or corrected vision.

  • Must have ability toclearly communicate verbally by phone or in person

Personal Protective Equipment:

  • Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.
