Job summary

Trinity Health is seeking a part-time Clinical Pharmacist to provide medication preparation, distribution, and pharmaceutical care within a hospital setting. The pharmacist will work on a rotating shift, collaborate with the healthcare team, and participate in CQI activities to enhance patient outcomes.

Required credentials include a Michigan pharmacy license (or eligibility) and a Doctor of Pharmacy degree; BLS/ACLS may be required per hospital policy. Prior hospital experience is preferred.

Qualifications

• Licensed pharmacist in Michigan or eligible
• BLS and/or ACLS certification required by hospital policy
• Hospital/clinical pharmacy experience preferred

Responsibilities

• Interpret and evaluate medication orders for appropriateness of therapy.
• Monitor patient medications for interactions, contraindications and adverse effects.
• Individualize therapy based on renal function, age, weight, labs, and pharmacokinetics.
• Collaborate with physicians and nurses to optimize drug therapy and interventions.
• Educate staff and patients about medication-related topics and safety practices.
• Direct and supervise pharmacy technicians and students in dispensing processes.
