Job summary

Qualifications

• Minimum 3 years of clinical pharmacy experience.
• Demonstrated leadership and management experience.
• Unrestricted North Carolina Pharmacist License required.
• Proficient with EMR systems and MS Word.

Responsibilities

• Provide one-on-one patient consultations on medications with provider collaboration.
• Coordinate care with physicians, nurses, and staff to optimize plans.
• Review patient histories to ensure safe medication use and reconciliation.
• Develop and monitor individualized medication plans.
• Educate patients on medications, side effects, and recovery options.
• Maintain accurate documentation of plans and progress.
• Monitor adverse drug events and report for safety improvements.
• Attend population health team meetings and conduct Medicare chronic care management.
