Qualifications

  • Minimum 3 years in clinical pharmacy.
  • Management and leadership experience.
  • Understanding of quality and healthcare initiatives.
  • Understanding of Case Management requirements for managed care contracts.
  • Proficient in EMR systems and Microsoft Word Suite.
  • Excellent written and verbal communication.

Responsibilities

  • Provide one-on-one patient consultations to discuss medications in collaboration with the provider.
  • Coordinate with physicians, nurses, and others to optimize care plans.
  • Review patient profiles for safe, effective use during transitions of care.
  • Develop, implement, and monitor individualized medication plans.
  • Educate patients on medication usage and potential side effects.
  • Maintain comprehensive documentation of plans and progress.
  • Monitor adverse events and contribute to safety improvements.
  • Attend population health team meetings and support adherence efforts.
  • Collaborate with the manager of population health and case management to achieve goals.
  • Stay competent in pharmaceutical best practices.
