Job summary

Qualifications

• Doctor of Pharmacy or PharmD equivalent.

• Credentials across multiple states with transfer/licensure requirements.

Responsibilities

• Collaborate with providers to develop and monitor individualized drug therapy plans.
• Evaluate patient responses to therapy and adjust regimens as needed.
• Provide direct patient counseling on medications and adherence.
• Review and verify prescriptions and prepare/compound medications
• Supervise technicians and ensure safe medication preparation and dispensing.
• Participate in safety, QA, and regulatory compliance activities.
• Mentor pharmacy interns and staff; stay current with therapies and practices.
