Indian Health Service in Oregon seeks a clinical pharmacist to provide direct pharmaceutical care for patients and manage select chronic disease cases, with responsibilities across therapy optimization, education, and regulatory compliance.

The role requires detailed drug therapy evaluation, patient interaction, and collaboration with healthcare teams to ensure safe, effective medication use within the IHS formulary.

Competenze

• 3 years of progressively higher level graduate education leading to a Ph.D., Ph.D., Pharm.D. or equivalent doctoral degree.
• One year of specialized experience at GS-11 level in Federal service with MTM, direct patient assessment, and pharmacotherapy recommendations.
• Education and/or experience must meet the qualification requirements within 30 days of the closing date.

Mansioni

• Monitor, modify, and evaluate drug therapy for optimal outcomes; obtain drug history and counsel patients.
• Perform pharmaceutical compounding for sterile/non-sterile preparations, including extemporaneous formulations.
• Evaluate medication orders for appropriateness considering guidelines, organ function, age, comorbidities, and formulary.
• Dispense medications with accuracy, labeling, safety, and regulatory compliance.
