Job summary

Qualifications

• Bachelor of Science degree in Pharmacy or PharmD Degree.
• Current registration with the Iowa Board of Pharmacy as a Certified Pharmacist.
• Ability to prioritize workload.
• Must possess excellent interpersonal and communication skills.
• Ability to work well with a team and interact with patients and healthcare providers.
• Required to be current in BLS per American Heart Association.
• Must complete Mandatory Reporter course by hire or within 6 months.
• Must adhere to regulatory standards and training requirements.

Responsibilities

• Delivers direct patient care and clinical practice.
• Maintains proficiency in decentralized pharmacy services.
• Collaborates with a multidisciplinary team to provide patient-centered care.
• Identifies high-risk patients and implements safety interventions.
• Makes evidence-based, patient-centered medication recommendations.
• Participates in pharmacy operations and medication dispensing.
• Assists with admission/discharge medication reconciliation as appropriate.
• Reviews orders for appropriateness, interactions, and allergies.
