Job summary

Estimated pay range: $54.17–$70.42 per hour, depending on experience.

Qualifications

• Bachelor of Science required.
• Doctor of Pharmacy (Pharm D) preferred.
• Progressive pharmaceutical care experience or Pharmacy Practice Residency preferred.

Responsibilities

• Provide pharmaceutical care through medication order review, preparation, dispensing, documentation and reconciliation.
• Collaborate with patients, physicians, nurses and other healthcare professionals to deliver safe, effective patient-centered care.
• Interpret and process medication orders, monitor therapy and provide therapeutic assessments.

Education

Bachelor of Science required

Doctor of Pharmacy (Pharm D) preferred

Pharmacy Practice Residency preferred
