Stormont Vail Health seeks a Clinical Staff Pharmacist to optimize medication therapy, prevent disease progression, and promote wellness across all ages. You will provide comprehensive pharmaceutical care, collaborate with the care team, and participate in education and process improvements.

Qualifikationen

• Bachelor's degree of Science in Pharmacy from an accredited school.
• Doctorate PharmD from an accredited school.
• 1 year Pharmacy practice experience as a licensed Pharmacist or PGY1 residency.

Aufgaben

• Review, interpret and verify provider medication orders with clinical interventions as needed.
• Complete pharmacy consults proficiently, following policies and procedures.
• Provide drug information to healthcare professionals and educate patients.
• Direct pharmacy technician workflow and verify final products prior to dispensing.
• Manage medication use including compounding, preparation, selection, dispensing, storage and stocking.
• Collaborate with the pharmacy team to ensure optimal patient care.
• Educate and train pharmacy personnel and serve as preceptors for students and residents.
• Develop and conduct medication use evaluations and report results to committees.
