Qualifications

• Pharm D. or BS Pharmacy with advanced training
• NH pharmacist license
• 1–3 years experience
• Pharmacy PG1 residency preferred

Responsibilities

• Review and monitor patient medication therapy for safety, efficacy, and appropriateness.
• Collaborate with physicians and healthcare providers on medication decisions and dosing.
• Provide clinical recommendations and document interventions.
• Prepare, label, verify, and dispense medications per physician orders and regulations.
• Ensure compliance with laws, accreditation standards, policies, and controlled substances rules.
• Maintain accurate dispensing records and oversee charging/crediting systems.
• Monitor medication-use processes and present improvement data.
• Oversee sterile and non-sterile compounding per USP guidelines.
• Supervise pharmacy technicians and support staff.
• Participate in staff orientation, training, and mentoring.
• Contribute to quality improvement and patient-centered care.
