Job summary

Hartford HealthCare is seeking a Pharmacist to manage medication therapy, supervise technicians, and advise nursing staff on appropriate drug administration. The role emphasizes safe, cost-effective and timely care across age groups.

Qualifications

• PharmD degree required.
• Current CT Pharmacist license or eligible within 45 days of hire.
• PGY-1 Pharmacy residency preferred.
• Prior experience with EPIC systems preferred.

Responsibilities

• Optimizes drug therapy for patients by designing appropriate, cost-effective regimens.
• May provide direct patient care for pain management through evaluations, recommendations, and follow-ups.
• Maintains safety of drug therapy by identifying adverse events and coordinating with care teams.
• Provides drug information and serves as a liaison between pharmacy and health care team.
• Investigates and resolves drug-related problems and conducts quality improvement efforts.
• Documents interventions and outcomes for benchmarking and continuity of care.
• May function as charge pharmacist as needed.
