Olathe Health System, Inc. is seeking a Clinical Pharmacist to provide comprehensive pharmaceutical care across hospital departments.

This part-time role will initially float among multiple areas with the potential to transition to full-time, requiring strong interdisciplinary communication and educational precepting abilities. The candidate should hold a PharmD degree, be licensed in the state, and may participate in ACLS/PALS/NRP and BLS as needed.

Qualifications

• Bachelor's Degree in Pharmacy from an accredited college or university.
• Completion of an accredited pharmacy post graduate training program (PGY-1 and/or PGY-2 residency program).
• Licensed Pharmacist - State Board of Pharmacy in state of primary practice site.
• ACLS, PALS/NRP, BLS certifications as necessary.

Responsibilities

• Participates in performance improvement efforts to enhance pharmaceutical care.
• Provides patient-focused pharmaceutical care by monitoring drug therapy for safety and cost effectiveness.
• Supports staffing by flexing to other areas to cover shifts as needed.
• Reviews medication orders for timely resolution and accuracy.
• Performs patient-specific medication reviews and adjusts doses for renal or hepatic dysfunction.
• Reports medication incidents and participates in safety reviews.
• Involves in medication reconciliation and patient education.
