Qualifications

• Bachelor's Degree in Pharmacy from an accredited college or university.
• Preferred completion of PGY-1 and/or PGY-2 residency.

Responsibilities

• Provide comprehensive pharmaceutical care and ensure safe, effective, and cost-conscious drug therapy.
• Actively participate in rounds and assess/monitor therapies for safety and efficacy.
• Support staffing and flex to other areas to cover supplemental shifts as needed.
• Perform medication profile reviews and document dosing adjustments for patient parameters.
• Report medication incidents and participate in safety and quality initiatives.
• Educate staff and precept residents and students; contribute to training and orientation.
