Qualifications

Graduate of an ACPE-accredited School of Pharmacy
BS Pharmacy (minimum) and/or Doctor of Pharmacy (preferred)
State of Michigan Pharmacist licensure - current/eligible
State of Michigan Controlled Substance license - current/eligible
Demonstrates sound professional judgment.
Knowledgeable of software applications related to the job (Word/Excel/PowerPoint; hospital CLIN apps; MAPS/MedWatch/VaccineWatch)

Responsibilities

Ensures safe, appropriate, cost-effective medication therapies for patients per policies/procedures.
Participates in development/implementation of clinical pharmacy services and treatment guidelines.
Provides education to practitioners, patients, and caregivers on pharmacotherapy.
Supervises pharmacy staff and ensures accurate dispensing and unit-dose operations.
Performs daily distributive pharmacist functions including verification of orders and IV admixtures.
Contributes to quality improvement and medication-use reviews.
Attends meetings and participates in departmental activities.
Maintains records and ensures safe handling of hazardous materials.
