Job Description Summary

The Clinical Pharmacist - Part-Time is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of Option Care, pharmacy practice professional standards, and applicable regulatory entities. This entry-level position introduces the practice of clinical pharmacy in the alternate setting, coordinating patient services with the Nursing Department, monitoring patient progress toward goals, and educating patients, caregivers, staff and other professionals regarding appropriate medication use.

Job Responsibilities
  • Determine the suitability of individual patients for home care, collecting and organizing all patient-specific information to assess suitability per Option Care policies.
  • Design, recommend, monitor and evaluate patient‑specific pharmacy care plans, making recommendations for modifications in medication therapy or monitoring parameters as appropriate.
  • Modify pharmacy care plans based on evaluation of monitoring data and other pertinent patient information, applying pharmacokinetic principles and laboratory value assessment.
  • Provide medication‑use education to Option Care staff, other professionals, home‑care patients, and/or their caregivers using effective patient education techniques.
  • Ensure continuity of pharmaceutical care to and from the home and other patient‑care settings, communicating pertinent patient information systematically.
  • Prepare and dispense medications following Option Care policies and procedures, maintaining safety, quality assurance, and compliance with state/federal laws and USP standards.
  • Manage the use, maintenance, and troubleshooting of medication‑administration equipment, completing quality assurance reports and monitoring inventory systems for controlled substances.
  • Respond to drug‑information requests from health‑care providers and patients, formulating evidence‑based answers and maintaining CEU compliance.
  • Manage investigational drug products in line with regulations and Option Care policies, and organize workflow for pharmacy support personnel.
  • Comply with accreditation, legal, regulatory, and safety requirements in all activities.
  • Maintain confidentiality of patient and proprietary information, observing legal and ethical guidelines.
Supervisory Responsibilities

No supervisory responsibilities.

Basic Education and/or Experience Requirements
  • Graduate of an accredited School of Pharmacy.
  • Current, active Pharmacy license as granted by the State Board of Pharmacy.
  • At least one of the following:
    • One year of experience in sterile IV admixture procedures.
    • Knowledge of non‑sterile compounding, immunizations and medication therapy management (MTM).
    • Completion of an ASHP‑accredited Pharmacy Residency Program.
    • Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
    • Equivalent experience approved by the VP of Pharmacy and/or SVP of Clinical Services.
Basic Qualifications
  • Reliable means of transportation to make home visits, including a valid driver’s license and proof of insurance if using a personal vehicle.
  • Experience providing customer service to internal and external customers, meeting quality standards and evaluating satisfaction.
  • Basic proficiency in Microsoft Word, Excel, and PowerPoint, including document handling, formatting, and basic functions such as spell check and printing.
Travel Requirements
  • Willing to travel up to 5% of the time for business purposes within state.
Preferred Qualifications & Interests (PQIs)
  • Completion of an ASHP‑accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience.
  • Experience in home care, hospital, or other clean‑room settings.
Management Requirements
  • At least one year of experience in indirect management of team members, including assistance in development, training, and assignment of work to other team members.

Salary will be determined by applicant’s education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Pay Range is $49.14–$81.91.
