Job summary

Option Care Health is seeking an entry-level Clinical Pharmacist to provide safe pharmacy services in the home care setting, coordinating with Nursing and other professionals. You will educate patients and caregivers, monitor progress toward goals, and ensure regulatory compliance.

The role requires a Pharmacist license and completion of an accredited program, with potential experience in IV admixture, immunizations, MTM, or residency training. Travel up to 5% within state may be required.

Qualifications

  • Graduate of an accredited School of Pharmacy.
  • Current, active Pharmacy license granted by the State Board of Pharmacy.
  • One of: at least 1 year sterile IV admixture experience; or non-sterile compounding, immunizations, MTM; or ASHP residency; or APPE; or equivalent experience approved by leadership.

Responsibilities

  • Determine suitability of patients for home care and collect information.
  • Design, monitor, and evaluate patient-specific pharmacy care plans; make medication therapy recommendations.
  • Modify care plans based on monitoring data and patient information.
  • Provide medication-use education to staff, patients, and caregivers.
  • Ensure continuity of pharmaceutical care and communicate patient information across settings.
  • Prepare and dispense medications, calibrate equipment, and apply safety practices.
  • Manage medication administration equipment and inventory systems.
  • Respond to drug information requests and maintain competency.
  • Use investigational drugs per regulations and policies.
  • Comply with accreditation, legal, and regulatory requirements.
  • Maintain confidentiality of patient and proprietary information.

Job Description Summary

The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with policy and regulatory standards. This entry-level position enters the practice of clinical pharmacy in the alternate setting, coordinating patient services with the Nursing Department, monitoring progress toward goals, and educating patients, caregivers, staff, and other professionals on medication use.

Job Responsibilities
  • Determine the suitability of individual patients for home care and collect all necessary information.
  • Design, recommend, monitor, and evaluate patient‑specific pharmacy care plans and make appropriate medication therapy recommendations.
  • Modify pharmacy care plans based on monitoring data and other patient information.
  • Provide medication‑use education to staff, patients, and caregivers.
  • Ensure continuity of pharmaceutical care and use systematic procedures to communicate patient information across care settings.
  • Prepare and dispense medications, calibrate equipment, and apply safety and quality assurance practices.
  • Manage medication administration equipment, troubleshoot issues, and maintain inventory systems.
  • Respond to drug information requests and keep professional competency up to date.
  • Use investigational drug products according to regulations and policies.
  • Comply with accreditation, legal, and regulatory requirements.
  • Maintain confidentiality of patient and proprietary information.
Supervisory Responsibilities

No supervisory responsibilities.

Basic Education and/or Experience Requirements
  • Graduate of an accredited School of Pharmacy.
  • Current, active Pharmacy license granted by the State Board of Pharmacy.
  • One of the following: (a) At least one year of experience in sterile IV admixture procedures; (b) knowledge of non-sterile compounding, immunizations and medication therapy management; (c) completion of an ASHP accredited Pharmacy Residency Program; (d) completion of an Advanced Pharmacy Practice Experience; or (e) equivalent experience approved by the Vice President of Pharmacy or Senior Vice President of Clinical Services.
Basic Qualifications
  • Experience providing customer service to internal and external customers, meeting quality standards and evaluating satisfaction.
  • Basic skill in Microsoft Word.
  • Basic skill in Microsoft Excel.
  • Basic skill in Microsoft PowerPoint.
Travel Requirements

Willing to travel up to 5 % of the time for business purposes within state.

Preferred Qualifications & Interests
  • Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience.
  • Experience in home care, hospital, or other clean‑room setting.
Management Requirements

At least one year of experience in indirect management of team members, including assistance in development, training, and assignment of work/projects to others.

Salary and Pay Range

Salary to be determined by education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data.
Pay Range: $97,113.10 – $161,862.67.
