Job Description Summary

Monday-Friday, day shift and on-call. The Clinical Pharmacist is a licensed professional who is responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of Option Care, pharmacy practice professional standards, and applicable regulatory entities. This entry-level position serves as a gateway into the practice of clinical pharmacy in the alternate setting.

Job Responsibilities (listed in order of importance and/or time spent)
  • Determines the suitability of individual patients for home care. Collects and organizes all patient-specific information needed to determine suitability. Assesses patients’ suitability in accordance with Option Care policies, considering their medical condition, prescribed medication therapy, and prognosis with clearly defined outcome goals.
  • Designs, recommends, monitors, and evaluates patient-specific pharmacy care plans for the pharmacotherapy of home care patients. Makes suitable recommendations for modifications in medication therapy or monitoring parameters when appropriate. Collects and organizes patient-specific information needed to prevent, detect, and resolve medication-related problems; identifies appropriate monitoring parameters and completes nutritional assessment.
  • Modifies the pharmacy care plans based on evaluation of monitoring data and other pertinent information. Demonstrates competency in pharmacokinetic principles, formulas, and laboratory value assessment.
  • Provides medication-use education to Option Care staff, other professionals, home care patients, and/or their caregivers. Uses effective educational techniques to counsel patients and caregivers.
  • Ensures continuity of pharmaceutical care to and from the home and other patient-care settings. Uses a systematic procedure to communicate pertinent patient information. Prepares and dispenses medications following appropriate techniques and policies, calibrates equipment, ensures appropriate concentration, compatibility, stability, and storage, and adheres to safety and quality assurance practices.
  • Manages the use, maintenance, and troubleshooting of medication administration and related equipment. Accurately programs, maintains, and advises others; participates in incident and error reporting; completes QAR documents; and monitors inventory discrepancies for controlled substances.
  • Provides concise, applicable, and timely responses to drug information requests from health-care providers and patients. Formulates responses based on literature analysis; attends educational seminars or participates in CEU opportunities to promote professional competency; complies with state-mandated CEU requirements.
  • Manages investigational drug products per regulations and Option Care policies; utilizes pharmacy support personnel effectively, prioritizes workload, organizes workflow, and ensures accurate work by support staff.
  • In all activities, complies with accreditation, legal, regulatory, and safety requirements.
  • Maintains confidentiality of patient and proprietary information while observing legal and ethical guidelines.
Supervisory Responsibilities

Does not include supervisory responsibilities.

Basic Education And/or Experience Requirements
  • Graduate of an accredited School of Pharmacy.
  • Current, active Pharmacy license granted by the State Board of Pharmacy.
  • One of the following: (a) At least one year of experience in sterile IV admixture procedures; (b) Knowledge of non-sterile compounding, immunizations, and medication therapy management; (c) Completion of an ASHP-accredited Pharmacy Residency Program; (d) Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy; or (e) Equivalent experience approved by the Vice President of Pharmacy and/or Senior Vice President of Clinical Services.
Basic Qualifications
  • Experience providing customer service to internal and external customers, meeting quality standards and evaluating customer satisfaction.
  • Basic proficiency in Microsoft Word.
  • Basic proficiency in Microsoft Excel.
  • Basic proficiency in Microsoft PowerPoint.
Travel Requirements

Willing to travel up to 5% of the time for business purposes within the state.

Preferred Qualifications & Interests (PQIs)
  • Completion of an ASHP-accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience.
  • Experience in home care, hospital, or other clean-room settings.
Management Requirements

At least 1 year of experience in indirect management of team members, including assisting in development, training, and assignment of work or projects to other team members.

Salary

Salary will be determined by the applicant’s education, experience, knowledge, skills, and abilities, as well as internal equity and market data alignment. Pay range: $102,221.94 – $170,369.91.
