Job summary

Chesapeake Regional Healthcare is seeking a clinical pharmacist to ensure safe provision of medications and participate in drug therapy oversight for patient care. The role includes reviewing prescriptions, counseling patients, and advising providers in a hospital setting.

The candidate should hold a Doctor of Pharmacy (PharmD) or BS in Pharmacy, be licensed in Virginia, and complete required certifications within six months.

Qualifications

• Bachelor of Science in Pharmacy required.
• Doctor of Pharmacy (PharmD) preferred; ASHP-accredited PGY1 pharmacy residency preferred.
• Licensure as a Pharmacist in Virginia required; BLS and ACLS within 6 months of hire.

Responsibilities

• Review and verify prescriptions and medication orders for appropriateness, safety and efficacy.
• Perform drug utilization reviews and intervene as needed.
• Manage pharmacotherapeutic services per standards/protocols.
• Monitor regimens for safety and efficacy; apply pharmacokinetic principles as warranted.
• Provide education to physicians, nurses, and pharmacy staff.
