Job summary

Chesapeake Regional Healthcare seeks a Clinical Pharmacy Specialist I to establish and coordinate pharmacotherapy services within designated specialties. You will proactively evaluate regimens, monitor safety, and intervene as needed, while collaborating with multi-disciplinary teams to advance patient care.

Educational activities and committee leadership are part of the role. The position emphasizes autonomy, evidence-based practice, and ongoing professional development in a hospital setting.

Qualifications

• Graduate from an ACPE-accredited College of Pharmacy.
• PharmD or higher preferred; ASHP-accredited PGY1/PGY2 in specialty area preferred.

Responsibilities

• Establish, coordinate, and maintain pharmacotherapy services for designated specialty patients.
• Prospectively evaluate regimens for indications, dosage, interactions, and allergies.
• Monitor safety and efficacy and intervene using pharmacokinetic principles as warranted.
• Lead or participate in hospital committees related to the area of practice.
• Educate physicians, APPs, nurses, and staff; conduct grand rounds and CE sessions.
• Collaborate with departments to ensure quality patient care and organizational mission.
• Develop and maintain policies and guidelines for medication use.
• Precept pharmacy residents and students as available.
• Develop evidence-based strategies to reduce adverse drug events.
• Serve as primary drug information resource within the hospital.
• Maintain current knowledge of drug use and primary literature.
