Qualifications

• Pharmacist License in good standing required within 120 days.
• Medication Therapy Services Certificate required within 120 days.
• Ability to travel between various system facilities as needed.

Responsibilities

• Evaluate patient specific care plans and resolve medication related issues.
• Ensure compliance with all pharmacist steps in the Medication Use Process.
• Supervise IV room activity to ensure patient safety.

Skills

Experience with unit-dose systems

IV Admixture programs

Computer applications to pharmacy services

Pre-packing/batch preparation/compounding skills

Education

Bachelor's Degree

PharmD from an accredited college of Pharmacy

30 hours of continuing education every 2 years
