Job summary

SUNY Downstate Health Sciences University is recruiting a full-time Pharmacist for its Department of Pharmacy in Brooklyn, NY. The role requires dispensing, counseling, medication reconciliation, and oversight of orders in the CPOE system and Pyxis profiles.

Ideal candidates hold a Pharm D. from an accredited college and may have PGY-1 residency experience. Responsibilities include supervising staff and covering holiday rotations, with a strong emphasis on safety, QA, and new technologies.

Qualifications

• Pharm D. from an Accredited College of Pharmacy.
• Completion of a PGY-1 Residency.

Responsibilities

• Dispense, compound, admix drugs and other pharmaceuticals.
• Provide medication counseling and reconciliation.
• Review medication orders for safety and efficacy and approve orders within our CPOE system to main medication profiles.
• Review and approve Pyxis profile orders.
• Administer oncology, TPN, and investigational drug solutions as needed.
• Dispense controlled substances as needed and maintain files.
• Participate in drug utilization review studies, QA, and PI programs.
• Actively participate in new technologies and operational programs.
• Maintain the records of Pharmacy.
• Supervise technicians and support staff.
• Report to Senior Pharmacists, the Director, and Associate Director of Pharmacy.
• Provide in-service education to healthcare professionals on demand.
• Rotate with medical teams and/or participate in specific medication management programs.
• Be available for holiday coverage rotation and on-call/recall rotation as required.
