Job summary

AdventHealth Sebring in Florida is seeking a licensed Pharmacist to educate and counsel patients, families, and the healthcare team on safe medication use while delivering high-quality care.

You will supervise technicians, participate in rounds, ensure safe transitions of care, monitor drug therapies, and contribute to quality initiatives. Florida license required; 1+ hospital experience preferred.

Qualifications

• PharmD degree or equivalent professional qualification.
• Licensed Pharmacist in Florida or eligible for licensure.
• 1+ year hospital pharmacy experience preferred.

Responsibilities

• Educate and counsel patients, families, and healthcare team on safe and effective medication use.
• Provide safe, timely, and cost-effective medication therapy; participate in stewardship and surveillance.
• Engage in direct patient care and interdisciplinary rounds; ensure transitions of care from admission to discharge.
• Supervise and mentor technicians, interns, students, and residents; support team learning.
• Identify, report, and help resolve medication safety events and ADRs.
• Review medication orders and patient profiles to prevent drug-related problems; perform pharmacokinetic dosing and monitoring.
• Contribute to quality improvement, regulatory compliance, and accreditation activities.

Job Description

• Educate and counsel patients, families, and healthcare team members on safe and effective medication use.
• Provide safe, timely, and cost-effective medication therapy; actively participate in stewardship, surveillance, and restricted-use programs.
• Engage in direct patient care and interdisciplinary rounds, ensuring smooth transitions of care from admission through discharge and follow-up.
• Supervise, mentor, and evaluate pharmacy technicians, interns, students, and residents; support team learning and development.
• Identify, report, and help resolve medication safety events, adverse drug reactions, and errors.
• Demonstrate proficiency with EHRs, pharmacy informatics systems, and medication automation to optimize clinical decision support and documentation.
• Contribute to professional advancement through practice innovation, research, specialty alignment (e.g., critical care, oncology, infectious disease), committee participation, and scholarly activity.
• Perform all pharmacist functions including compounding, dispensing, verification, and documentation of medications across central, satellite, and IV areas.
• Review and monitor medication orders and patient profiles to prevent, identify, and resolve drug-related problems; complete pharmacist consults and perform pharmacokinetic dosing, monitoring, and adjustments.
• Other duties as assigned.
• Participate in quality improvement initiatives, regulatory compliance, and accreditation activities; track performance metrics (e.g., interventions, error reduction, cost savings).
