Job summary

AdventHealth is seeking a pharmacist to educate and counsel patients and healthcare teams on safe medication use in a full-time hospital setting in Winter Haven, FL. The role includes direct patient care, rounds, and interdepartmental collaboration to ensure safe transitions of care.

The position requires a PharmD with a Bachelor’s degree as a minimum and licensure as a pharmacist; inpatient experience is preferred. Comprehensive benefits apply and support for career development is provided.

Qualifications

  • Educate and counsel patients, families, and healthcare team on medication use.
  • Provide safe, timely, and cost-effective therapy; participate in stewardship and related programs.
  • Engage in direct patient care and interdisciplinary rounds; ensure transitions of care.
  • Supervise, mentor, and evaluate pharmacy staff, interns, students, and residents.
  • Identify, report, and help resolve medication safety events and errors.
  • Demonstrate proficiency with EHRs and pharmacy informatics systems; support practice innovation.
  • Perform all pharmacist functions including compounding, dispensing, verification, and documentation.
  • Review and monitor medication orders to prevent drug-related problems and adjust dosing.
  • Participate in quality improvement, regulatory compliance, and accreditation activities.

Job Description

  • Educate and counsel patients, families, and healthcare team members on safe and effective medication use.
  • Provide safe, timely, and cost-effective medication therapy; actively participate in stewardship, surveillance, and restricted-use programs.
  • Engage in direct patient care and interdisciplinary rounds, ensuring smooth transitions of care from admission through discharge and follow-up.
  • Supervise, mentor, and evaluate pharmacy technicians, interns, students, and residents; support team learning and development.
  • Identify, report, and help resolve medication safety events, adverse drug reactions, and errors.
  • Demonstrate proficiency with EHRs, pharmacy informatics systems, and medication automation to optimize clinical decision support and documentation. Contribute to professional advancement through practice innovation, research, specialty alignment (e.g., critical care, oncology, infectious disease), committee participation, and scholarly activity.
  • Perform all pharmacist functions including compounding, dispensing, verification, and documentation of medications across central, satellite, and IV areas.
  • Review and monitor medication orders and patient profiles to prevent, identify, and resolve drug-related problems; complete pharmacist consults and perform pharmacokinetic dosing, monitoring, and adjustments.
  • Other duties as assigned.
  • Participate in quality improvement initiatives, regulatory compliance, and accreditation activities; track performance metrics (e.g., interventions, error reduction, cost savings).
