MedWatchers, Inc. is seeking a Clinical Pharmacist specializing in MTM to join our remote MTM call center. You’ll conduct telephonic MTM reviews, engage with patients and caregivers, and collaborate with prescribers to optimize therapy.

What You'll Do

• Conduct telephonic CMRs and TMRs with patients and caregivers, following the 5 Core Elements of MTM (Medication Therapy Review, Personal Medication List, Medication-Related Action Plan, Interventions/Referrals, and Documentation & Follow-up).
• Identify medication-related problems, adherence barriers, and gaps in care; make evidence-based recommendations.
• Collaborate with prescribers, pharmacies, and care teams to optimize therapy.
• Document all interventions accurately in our clinical platform, in full compliance with HIPAA and state/federal regulations.
• Provide clear, empathetic patient education over the phone.
• Support quality improvement across clinical and operational workflows.
