The Massachusetts General Hospital is looking for motivated individuals to join our growing team in the ambulatory care space.
Pharmacists play a key role by offering focused services (medication prescribing, monitoring, education) to patients with chronic diseases. Working with primary care providers, population health coordinators, high-risk nursing programs, and community health workers, the pharmacists positively impact patients through virtual and in-person appointments. Clinical Pharmacists serve as a departmental resources and liaisons to other departments, hospital personnel, or external groups. They proficiently manage and facilitate the appropriate use of medications according to professional and regulatory standards. They also conduct clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. Clinical Pharmacists provide medication and practice-related education/training and actively serve as preceptors for pharmacy residents and Doctor of Pharmacy students.
As a member of the primary care team, the Ambulatory Care Clinical Pharmacist will be responsible for the provision of safe and effective patient centered pharmacotherapy in the primary care space. Clinicians are aligned with targeted interdisciplinary programs and specialty services to manage chronic disease states through Collaborative Drug Therapy Management agreements (CDTM).
Licenses, Certifications, and/or Registrations: Current licensure as a pharmacist in the Commonwealth of Massachusetts (or eligible) required. Board Certification in area of expertise (BCACP, CDE) is required within 18 months of hire (if not already certified).
Education: Graduate of an ACPE accredited College of Pharmacy required. Pharm.D. or other advanced degree preferred. Completes required additional continuing education requirements including at least one hour of pain training every 2 years and 5 contact hours of CE in the area of credentialing yearly. Maintains active credentialing status, MA controlled substance license, and completes all required documentation timely.
Experience: Completion of ASHP accredited PGY1 Pharmacy Residency required; PGY-2 Ambulatory Care residency preferred, or equivalent experience. Clinical pharmacy experience (1-5 years) in ambulatory care settings and/or CDTM experience preferred; required if no PGY2 residency training. Exceptional aptitude or knowledge may be considered in lieu of experience. Extensive clinical experience and development may be considered in lieu of formal training.
Physical Requirements: Standing frequently, walking frequently, sitting occasionally, lifting frequently (35 lbs+ with assistance), carrying 20-35 lbs, pushing and pulling occasionally, climbing rarely, balancing frequently, stooping, kneeling, crouching, crawling, reaching, handling, fingering, feeling, foot use, and visual and auditory requirements as detailed.
