Responsibilities

• Provide medication order verification and medication therapy monitoring.
• Assess optimal drug product selection, dosage regimen design, and effective administration.
• Interact with patients, families, and healthcare providers to assure continuity of care and desired patient outcomes.
• Supervise pharmacy technicians and assume essential pharmacy management responsibilities when necessary.
• Lead pharmacy services in a clinical or operational area of the health system.
• Provide expertise and oversight to designated area and staff.
• Ambulatory Care: complete comprehensive medication reviews and adjust meds per collaborative practice.
• Identify and deprescribe unneeded or harmful medications; provide education and clinical expertise.
