Job summary

PharMerica in Sacramento, CA, is hiring a full-time Pharmacist for a non-retail, closed-door setting serving long-term care and senior living clients. Overnight schedule 7 on/7 off with a comprehensive benefits package.

You will evaluate orders, provide clinical recommendations, maintain patient profiles, monitor labs, and collaborate with physicians and nurses to ensure safe, effective medication therapy and regulatory compliance.

Qualifications

• Bachelor’s degree in Pharmacy required; PharmD preferred.
• 1 year experience in long-term care desired.
• Current pharmacist license in state of practice.
• Knowledge of drug therapy guidelines, medication safety, and regulatory standards (CMS, DEA, FDA).
• Proficiency in electronic medical records and pharmacy information systems.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, correct dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and staff regarding therapy changes.
• Maintain up-to-date patient/resident profiles incl. medical history, labs, meds.
• Ensure monitoring labs are ordered/reviewed for high-risk meds.
• Collaborate with physicians, nurses, and other healthcare professionals.
• Document communications and interventions in patient records.
• Implement and enforce policies for safe handling, dispensing, and management of meds.
• Ensure compliance with state/federal regulatory requirements.
• Report medication-related errors to Pharmacy Manager.
• Educate clinical team and staff on safe medication handling procedures.
• Perform other pharmacy-related tasks as assigned.
